Please follow the link below to download the declaration and the Bankers Order Form. You must print off these documents and fill them in before sending them to College. You will need Adobe Reader to view the documents.

1. Visit http://www.st-mary.blackpool.sch.uk and click the Documents link on the left hand menu.
2. On the page that loads login as a guest by clicking the link at the bottom.
3. Click the Parents link on the list of folders.
4. Click and download the Bankers Order Form and the Gift Aid Declaration Form.
5. Print out these documents to fill in before sending them into College.
